First Visit

Your initial appointment may consist of a consultation and/or treatment. Factors such as a complex medical history or a complex treatment plan may require you to initially be seen for a consultation only. Treatment would be scheduled at a later date, following the consultation. Please contact our reception team if you have any questions regarding your appointment.

Please assist us by providing the following information at the time of your consultation/treatment appointment:

  • Any medical concerns or medical conditions, along with a list of medications you are presently taking;
  • If you have medical or dental insurance, we can provide you with forms to send to your insurance;
  • Your provincial health card;
  • Proof of treaty status if applicable.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or legal guardian at the consultation or first visit.

If you are having conscious IV sedation for your treatment, you are required to have had nothing to eat or drink for eight hours prior to your surgery. A responsible adult is asked to remain in the office for the duration of your appointment and drive you home.

X-Rays

We will require x-rays at the time of your consultation/treatment. We often receive x-rays with your referral from your dentist, however, we may require a current x-ray to ensure the best treatment is provided. X-rays can be taken in our office if needed.
