Referrals – Please be mindful of the three different referral options listed below.

Are you experiencing technical difficulties attaching files to your SOS or SOMOP referrals? Please scroll to the bottom of this page for troubleshooting assistance. If you continue to experience difficulties after troubleshooting, you can send a referral online as per usual, but email your attachments to [email protected]. Please ensure you provide the patient’s name, xray dates and date you sent the referral. Thank you!
